If you have been researching medical weight loss, you have probably encountered two names repeatedly: semaglutide and tirzepatide.

Both are injectable medications used in modern weight-management care. Both influence appetite-related signaling. Both can produce meaningful weight reduction in appropriately selected patients.

But they are not the same medication.

Semaglutide primarily acts through the GLP-1 receptor. Tirzepatide acts through both GLP-1 and GIP receptors, giving it a different pharmacologic profile.

A 2025 head-to-head clinical trial in adults with obesity without diabetes found greater average weight reduction with tirzepatide than semaglutide over 72 weeks. That does not mean tirzepatide is automatically the correct choice for every patient. Medical history, tolerability, contraindications, treatment goals, medication availability and provider judgment all matter.

At Avendano Health in Boca Raton, both options are available through clinician-directed 12-week Medical Weight Loss programs, subject to medical evaluation and provider authorization.

FDA-approved Wegovy contains semaglutide and is indicated for chronic weight management in qualifying patients. FDA-approved Zepbound contains tirzepatide and is also indicated for chronic weight management in qualifying patients.

However, patients should understand an important distinction: a compounded medication is not an FDA-approved drug, even when it involves an ingredient also found in an FDA-approved product.

The exact medication, formulation and dose used in an Avendano Health program are determined by the prescribing provider when clinically appropriate.

How Does Semaglutide Work?

Semaglutide is a GLP-1 receptor agonist.

GLP-1 is involved in appetite, satiety, glucose regulation and gastric emptying. By activating this pathway, semaglutide can reduce appetite and help some patients feel satisfied with less food.

That can make maintaining a calorie deficit more manageable.

It does not eliminate the importance of nutrition, physical activity, sleep or other factors affecting long-term weight management.

How Is Tirzepatide Different?

Tirzepatide activates two pathways:

GLP-1 + GIP.

That dual mechanism is the major pharmacologic difference.

In the SURMOUNT-5 head-to-head trial, participants receiving maximum tolerated tirzepatide doses lost an average 20.2% of baseline weight at 72 weeks compared with 13.7% among participants receiving semaglutide.

Those are study averages—not promises for an individual patient.

The trial also evaluated specific FDA-approved medications, doses and a defined study population. Its results should not be represented as guaranteed outcomes from a compounded 12-week program.

Does Tirzepatide Work Better Than Semaglutide?

On average, tirzepatide produced greater weight reduction in the major head-to-head trial.

But “Which drug produced more average weight loss?” and “Which treatment should I use?” are different questions.

A provider may consider factors such as:

  • medical history,
  • previous GLP-1 use,
  • gastrointestinal conditions,
  • diabetes medications,
  • gallbladder or pancreatic history,
  • kidney issues,
  • pregnancy or pregnancy plans,
  • personal or family history relevant to medication warnings,
  • current medications,
  • previous side effects,
  • treatment goals.

That is why Avendano's pathway begins with a medical evaluation rather than letting the patient simply choose a medication based on an advertisement.

What Side Effects Should Patients Know About?

Both medications commonly produce gastrointestinal side effects.

Depending on the medication, these can include:

  • nausea,
  • diarrhea,
  • vomiting,
  • constipation,
  • abdominal discomfort,
  • indigestion,
  • reflux,
  • decreased appetite.

More serious risks and contraindications also exist.

FDA-approved semaglutide and tirzepatide weight-management products carry boxed warnings concerning thyroid C-cell tumors observed in rodents and are contraindicated in patients with a personal or family history of medullary thyroid carcinoma or MEN 2.

Other warnings vary by medication and can include pancreatitis, gallbladder disease, kidney injury related to dehydration and other clinically significant concerns.

This is one reason treatment eligibility should be determined medically rather than through an online quiz alone.

Semaglutide or Tirzepatide: How Do You Choose?

You do not need to diagnose yourself.

A useful starting question is:

“Am I a candidate for medical weight-management treatment, and if so, which option makes the most sense for my health history?”

That is the role of the medical evaluation.

If you are comparing semaglutide and tirzepatide because you are ready to take the next step, review Avendano Health's Medical Weight Loss programs, see the complete program price before enrollment and then complete the eligibility process.
